To become a junior member please follow the instructions bellow, or feel free to contact us or visit us on campus, and we will arrange your membership.

1.  Open and print the the application.

2.  Fill the application out, and send it to the address listed below. Be sure to include a check or money order for $10 to cover you dues for the year. Remember that ten percent of all money raised (including membership dues) goes toward the Tony Leonardi Scholarship in Jazz.  Make checks payable to the Tony Leonardi Double Bass Association.  Thank you.
